Your information
Website privacy notice
How we handle website appointment requests. Clinical consent and nursing records are managed separately.
Updated October 2, 2026
What the form sends
The form asks for your contact name, phone number, optional email, who the request is for, municipality, the first three characters of your postal code and preferred appointment times. It also sends the displayed visit rate and your contact consent. Please provide your own contact details if arranging care for another adult. Do not enter their medical details or health card number.
How your request reaches us
When you submit, your browser sends the request to FormSubmit, which emails it to SureStep’s designated Gmail inbox. FormSubmit may ask you to complete a spam-prevention check. After a successful submission, you are directed to SureStep’s thank-you page. The website itself has no clinical-record database, payment form or file-upload facility.
FormSubmit and Google are external service providers. Information may be processed outside Canada. Email is not a confidential clinical messaging system; interception, misdelivery or unauthorized account access can expose information. A successful web submission confirms acceptance by the form service, not that a staff member has read it or that an appointment is booked.
Read FormSubmit’s privacy terms and Google’s privacy policy.
Your choice and consent
Use the form only if you agree to this processing for the purpose of responding to your appointment request. You may call 289 501 9908 instead. Consent to contact is separate from consent to nursing treatment, which is discussed with the client or authorized decision-maker before care.
Health information and clinical records
Please discuss health history, medications, symptoms and other care details directly with the nurse through an agreed clinical intake process. Do not send these details by ordinary email. Website appointment information can still be sensitive because it relates to a request for nursing care.
Clinical records require a separate documented process for authorized access, storage, retention, secure disposal and privacy incidents. This website does not establish or certify that process, and does not claim certification under Ontario’s Personal Health Information Protection Act (PHIPA).
Access, corrections and concerns
To ask about your request, correct contact details, withdraw contact consent or request deletion subject to applicable retention obligations, call 289 501 9908 or email mrsfinest24@gmail.com. Email only basic contact details; do not include medical information. Provider-held copies and retention are also subject to the external services’ practices.
You can raise a health-information privacy concern with the Information and Privacy Commissioner of Ontario.
